Pterygoid implants
The pterygoid implant is placed in the posterior region of the upper jaw and is connected to other implants in the anterior maxillary area, serving as a strategic support point.
This implantology technique is a variation of the tilted implant approach.
It is an implantology technique that can be considered a variation of tilted implants, as they are used when there is severe atrophy in the maxillary bone. Combined with implants in more anterior areas, they allow for fixed prosthetic treatments with implants without the need for extensive bone grafting.
Given the evolution of implantology processes, this technique is highly useful today as an alternative to certain bone grafts, as it reduces treatment times, the number of surgical interventions, and patient discomfort.
A radiological study and a specialist evaluation are highly recommended to determine the suitability of undergoing this treatment. This type of implant is indicated for patients with deficient bone quality and quantity in the posterior regions of the maxilla, excessive pneumatization of the maxillary sinus, or insufficient bone width or height in the alveolar ridge—conditions that prevent the placement of conventional implants unless bone grafting is performed.

How the pterygoid implant technique is performed
Pterygoid implants are placed through a straightforward surgery under local anesthesia. The main difference between this technique and conventional implants is the use of long implants, with a minimum length of 16-20 millimeters, to reach the pterygoid process. The implant is tilted towards the posterior area to reach the apophysis region, where sufficient bone can be found to anchor an implant even in cases with significant bone loss in other areas. It is common to use osteotomes (punch-like instruments) in combination with standard dental drills; these provide greater control and allow the specialist to condense the existing bone rather than removing it while creating the implant site.
Advantages of pterygoid implants
Pterygoid implants are an alternative technique to the well-known sinus lift. They offer two major advantages over that procedure: first, the placement of pterygoid implants preserves the integrity of the maxillary sinus, making them suitable for patients with sinus pathologies where a sinus graft would be contraindicated. Additionally, pterygoid implants are anchored in the patient’s own bone, which reduces overall treatment time compared to the sinus lift technique, which requires waiting several months for the bone graft to consolidate.
Pterygoid implants are indicated for edentulous patients in the posterior segments who can receive implants in the premolar area but lack sufficient bone in the molar region.
This approach allows the patient to have fixed teeth by placing a bridge between the standard premolar implants and the pterygoid implant.
They are also indicated for fully edentulous patients as a powerful reinforcement element that allows for the fabrication and positioning of fixed prostheses, as well as for patients seeking a shorter implant treatment timeframe.
